Information Release Form

I do hereby give Owens Community College and its representatives the right to release and discuss any and all information pertaining to my performance at Owens Community College to/with my sponsoring dealership or service center. This includes, but is not limited to: test grades, final grades, attendance, work ethic, etc. I understand that this release of information will stay in effect during my entire enrollment at Owens Community College.
