Request Official Transcripts

Request official transcripts online through Ozone. Select Transcript from the Quick Links.

Former Students
Complete the Transcript Request Form and return it in person to Oserve or by mail or fax to the Records Office. Official transcripts may also be requested in the form of a letter containing all required pieces of information. Mailed transcript requests should be addressed to Owens Community College Records Office P.O. Box 10,000 Toledo, OH 43699. Transcript requests can be faxed to (567) 661-2101. E-mail requests cannot be accepted as your signature is required.

Official transcripts must be ordered in advance. Official transcripts are not available on-demand. Processing usually takes three business days. Please allow additional processing time at the beginning and end of each semester when we experience a high request volume.

To avoid delays, be sure the Transcript Request Form is filled out completely and all holds and financial delinquencies are cleared.

There is no charge for transcripts however, transcripts are withheld if all financial obligations to the College are not fulfilled.

The following information is required in order to process your transcript request:

  1. Full name, typed or printed (please include prior name(s) you may have used at Owens)
  2. Social Security Number or OCID (Owens College ID number)
  3. Current address
  4. Telephone number
  5. The destination mailing address. If no destination address is provided, your transcript cannot be processed.
  6. Your signature
