To create your Owens employee profile, we ask that you complete the following forms using the links provided below. You will be able to edit and digitally sign each form and will be prompted to “submit” each document when you are done. A final copy of the documents will be sent to Human Resources and to your personal email address when complete.

SERS Exemption Form

* All non-faculty, regular full- or part-time employees are required to have a minimum amount of retirement funds withdrawn from their paycheck and deposited in either the State of Ohio’s program (SERS) or an alternative (private) retirement fund. As a student, you have the option to exempt yourself from the SERS Retirement Program. This form will prevent the automatic withdrawal of your paycheck for the SERS Program.

Note: On your first day of employment, you will be required to bring two forms of identification into Human Resources. These documents are utilized in your Employment Eligibility Form (Form I-9). Identification needs to be active and examples include: driver’s license, state identification card, passport or passport card, permanent resident card, social security card, or a birth certificate. Documents provided must be in good condition and unexpired. Please contact Human Resources if you are unsure about what documents you can provide.
