This form is for students who have been suspended from Owens Community College for disciplinary reasons. Once your suspension period has ended, if you would like to return to Owens, please fill out this form and click submit at the bottom of the page. Once we receive your readmission consideration request form, Student Conduct will reach out to you regarding next steps. (This form must be submitted at least 4 weeks prior to the start of the semester you would like to return.)
