If you’d like to make an appointment, please go to the Counseling Services card in Ozone and complete the electronic forms listed there.

To find Counseling Services Forms in Ozone

1. Log in to your Ozone account.
2. Click on the Counseling Services card in your Ozone account.
   1. If you don’t see the Counseling Services card, scroll to the bottom of the Ozone homepage and click the DISCOVER MORE button
   2. Type Counseling in the search bar and select Counseling Services
   3. To save the Counseling Services card as a favorite, click the Save Card icon in the bottom left of the card
